Referring a child for a wish is one of the most important parts of helping the Foundation accomplish its mission of granting wishes to children with life-threatening medical conditions.

Refer a Child in Oregon or Clark County, Washington
If you are the parent or legal guardian of a child who has reached the age of 2 1/2 and is under the age of 18  who lives in Oregon or Clark County, Washington, please fill out the form below or call our office at 503-292-2280 ext. 10 to begin the wish process. Your information will be forwarded to the Make-A-Wish Foundation of Oregon, and you will be contacted shortly with more information about the referral process. Potential wish children or their attending medical professionals may also request more information.

Refer a Child who Lives in Another State than Oregon
